Crelate’s Onboarding Checklist allows for the capture of legal HR data. This may be different than the information gathered during the recruiting process or provided when a candidate applied to a role.

  • Legal First Name

  • Legal Middle Name

  • Legal Last Name (Required)

  • Street (Required)

  • Street 2

  • City (Required)

  • State (Required)

  • ZIP Code (Required)

  • Phone Number (Required)

  • Date of Birth (Required)

  • U.S. Social Security Number (Required)

  • Emergency Contact First Name (Required)

  • Emergency Contact Last Name (Required)

  • Emergency Contact Phone Number (Required)

This form requires the employee acknowledge the Crelate Terms of Service and provide an electronic signature.

Data entered into the Employee Profile can be viewed in the HR Center.

Navigate to Settings | Checklist Templates |Onboarding Checklists and select or create the desired onboarding checklist to add the Employee Profile requirement.

Select Add Item | Onboarding Actions | Employee Profile

Note – The Employee Profile is required in order to include State Tax Forms in the Onboarding Checklist.
